Enbucrilate, a rapidly polymerizing cyanoacrylate adhesive, was employed in the targeted occlusion of placental vascular anastomoses in a case of twin-twin transfusion syndrome (TTTS) diagnosed at 26 weeks of gestation. Under real-time color Doppler ultrasonographic guidance, enbucrilate was selectively injected into the interfetal vascular connections identified on the chorionic plate. The polymerization of enbucrilate upon contact with blood provided immediate and effective embolization, leading to cessation of aberrant transfusion between the twins.
This minimally invasive intervention was performed with precise localization of anastomoses and control of adhesive delivery to avoid collateral tissue damage. The procedure allowed stabilization of the fetoplacental hemodynamics, and the pregnancy progressed uneventfully to 35 weeks' gestation. Delivery via cesarean section resulted in two viable male neonates weighing 2,470 g and 1,750 g, respectively. This case highlights the clinical potential of enbucrilate as an effective embolic agent in fetal therapy. Its rapid solidification and strong adhesive properties enable precise vascular occlusion, offering a non-surgical alternative to laser ablation techniques. Enbucrilate-mediated vascular occlusion under Doppler guidance thus represents a valuable strategy for improving outcomes in monochorionic pregnancies complicated by TTTS.
