Treatment guidelines recommend a single dose of benzathine penicillin G (BPG), but real-world data supporting this regimen in the HIV population have been limited. This study highlights the clinical application of Penicillin G Benzathine as an effective single-dose therapy for early syphilis in HIV-infected patients, validating current treatment guidelines.
This retrospective analysis examined over 400 cases of early syphilis within a military HIV cohort. Treatment response was rigorously defined as a four-fold or greater decline in nontreponemal antibody titer at 13 months post-treatment. The study directly compared outcomes between patients receiving the standard single 2.4 million unit dose of BPG and those receiving multiple doses.
Key Clinical Findings:
· Equivalent High Efficacy: The treatment response rate was identical-92%-for patients receiving a single dose of BPG and those receiving more than one dose. Statistical analysis confirmed the number of BPG doses did not affect the serological response.
· Guideline Validation: The results strongly support existing recommendations, confirming that a single intramuscular dose of BPG is sufficient for treating early syphilis in HIV-infected persons.
· Associated Response Factors: While the BPG regimen was equally effective, the speed of serological response was influenced by patient-specific factors; higher baseline antibody titers and higher CD4 counts were associated with a faster titer decline.

Penicillin G Benzathine (BPG) is the cornerstone for secondary prophylaxis to prevent recurrent rheumatic fever (RF) and rheumatic heart disease (RHD). Effective dosing requires maintaining penicillin G concentrations above the target threshold.
Solution & Application
This study developed and validated a novel dried blood spot (DBS) assay to facilitate pharmacokinetic research for BPG. The assay uses a simple finger-prick sample (20 µL of blood) spotted onto filter paper, which is stable for transport and storage. It was rigorously validated against standard plasma assays in patients receiving various penicillin formulations. The assay demonstrated excellent correlation with plasma concentrations in patients receiving intramuscular BPG or bolus intravenous penicillin, confirming its reliability as a surrogate for pharmacokinetic studies.
Key Performance & Validation
· High Sensitivity: The assay achieved a limit of quantification of 0.005 mg/L, well below the target therapeutic threshold of 0.02 mg/L for Group A Streptococcus.
· Strong Correlation: For patients on BPG, plasma and DBS penicillin G concentrations showed an excellent correlation (Spearman's rs = 0.98), with comparable time-concentration profiles.
· Robust Stability: Penicillin G in DBS samples was stable for approximately 12 hours at room temperature (22°C), 6 days at 4°C, and over 1 month at -20°C, making it suitable for field studies.

Penicillin G Benzathine is the long-acting benzathine salt form of penicillin G, a broad-spectrum penicillin antibiotic. It binds to penicillin binding proteins (PBP) and inhibits bacterial cell wall synthesis.
Penicillin G benzathine inhibits bacterial cell growth by binding to penicillin binding proteins (PBP), which are enzymes involved in the synthesis of peptidoglycan, a critical component of the bacterial cell wall. This interruption of cell wall synthesis leads to bacterial cell growth inhibition and cell lysis.
The EC number of Penicillin G benzathine is 804-898-2.
Penicillin G benzathine is prepared by combining the sodium salt of penicillin G with N,N'-dibenzylethylenediamine to create a semisynthetic antibiotic.
